APPLICANT AGREEMENT
Please indicate your agreement to the following statements. You will also be asked to sign a confirmation document in person if you are selected as a Leadership Lee's Summit participant.

I understand attendance at the September & April sessions is mandatory. *

If selected as a participant, I am willing to attend all functions sponsored by the program. *

I understand if I miss more than 8 hours, I will need to make up in community service hours. *

I understand I may be ineligible to graduate if I miss more than TWO sessions. *

I understand I will participate in a group project that will require time outside regular sessions. *

I understand I represent my organization and will conduct myself professionally.

I understand if I fail to meet expectations, I may be asked to withdraw or not graduate the program. *
